PLEASE NOTE: Yesterday, the City of Collinsville was made aware of an issue regarding the QR code/link on the front of the Collinsville Community Survey postcard that was mailed to a randomly-selected group of residents as part of the National Community Survey.

Per our representative from Polco (the organization that administers the National Community Survey): If you were in the group of residents who were randomly-selected to receive the National Community Survey, please use the link that was on the back of the postcard instead of the QR code that was on the front of the postcard.

As a quick reminder: As this part of the National Community Survey was only mailed to a randomly-selected group of residents, the link is not meant to be shared with the general public at this time. The general public will have a chance to provide input in the coming weeks.
