PLEASE NOTE: The City of Collinsville has been made aware of a potential issue regarding the QR code/link on the Collinsville Community Survey that was mailed to a randomly-selected group of residents as part of the National Community  Survey.
 

We have contacted our representative at Polco, the organization that administers the National Community Survey and are awaiting their response with how residents should move forward. We expect to hear back after the weekend.
 

If you were in the group of residents that received a mailed survey, please hang tight until we receive an update before completing the survey. We will post an update on here as soon as we have one.
